Frozen Shoulder Recovery: What to Expect at Each Stage

Frozen shoulder, medically known as adhesive capsulitis, is a condition characterised by stiffness, pain, and a significant loss of motion in the shoulder joint. It typically develops gradually and can last anywhere from one to three years if left untreated. Understanding the stages of frozen shoulder can help you manage expectations and stay consistent with your recovery.

Stage 1 — The Freezing Stage (Painful Phase)

This stage can last anywhere from 6 weeks to 9 months. You'll notice a slow onset of shoulder pain that worsens with movement and at night. As the pain increases, your range of motion begins to decrease. At this point, the shoulder joint's capsule is becoming inflamed and thickening. Pain management and gentle movement are the focus during this phase.

Stage 2 — The Frozen Stage (Stiff Phase)

Lasting between 4 to 6 months, this is the stiffest phase. Interestingly, the pain may slightly reduce compared to Stage 1, but shoulder movement becomes severely restricted. Daily activities like reaching overhead, behind your back, or even lifting a glass can become difficult. Physiotherapy during this stage focuses on maintaining what range of motion remains and preventing further stiffness.

Stage 3 — The Thawing Stage (Recovery Phase)

This is the most encouraging phase, lasting anywhere from 6 months to 2 years. Shoulder movement gradually returns as the capsule begins to loosen. With consistent physiotherapy, stretching, and strengthening exercises, most patients regain significant — and often full — range of motion. Active rehabilitation is critical during this stage to speed up recovery.

How Physiotherapy Helps at Each Stage

A skilled physiotherapist tailors treatment to your specific stage. In the freezing stage, the focus is on pain relief using techniques like electrotherapy, manual therapy, and dry needling. During the frozen stage, gentle joint mobilisation and stretching help maintain mobility. In the thawing stage, progressive strengthening and functional movement training restore full shoulder use.

Tips to Support Your Recovery

Consistency is everything with frozen shoulder. Perform your prescribed home exercises daily, apply heat before stretching to warm up the joint, and avoid activities that cause sharp pain. Sleep position matters too — avoid lying directly on the affected shoulder.
